Ways to report a suspected side effect of drugs:

1. Online - CLICK THIS BOX

2. Download and print off a patient Yellow Card form from the MRHA website and complete it by hand (no postage stamp is needed).

3. Pick up a Yellow Card form from your GP surgery or pharmacy.

4. Call the Yellow Card hotline on 0808 100 3352 to request a form.
